Southern California Integrated Care Network – Inland Empire (SCICN-IE) has expanded each year since its inception in 2012. Its IEHP DualChoice Cal Mediconnect Plan launched in 2014, and for 2015–2017, gainsharing totaled more than $354,000. The network’s BPCI Model 2 also began in 2014, with gainsharing totaling over $400,000 from Q2 2014 through Q2 2017. The network’s Blue Shield and HealthNet contracts launched in 2014, and SCICN-IE continued to grow in 2019 through contracts with United and Anthem.
The network has been working with participating primary care offices to close gaps in care. That includes making sure patients are being informed if they are due for preventive services. In addition, the team is reaching out to practices with patients who are high utilizers of the ER or have frequent hospital readmissions.
Quality Highlight
While SCICN-IE continues to expand, it’s maintaining a strong focus on quality improvements. The network recently launched an initiative to support physicians in addressing the high incidence of patients with uncontrolled diabetes in the area. The network also increased patient-provider engagement through care coordination and educational resources.
Its care coordination program has increased patient engagement, and the program has a 99.1% positive experience score as reported through patient satisfaction surveys.
