Saint Joseph Health Partners (SJHP) is the largest accountable care organization (ACO) in Kentucky, managing more than 100,000 lives with a network of providers that span the state and include hundreds of care delivery points, from hospitals and skilled nursing facilities to ambulatory care centers and clinical offices. To support its growth, SJHP has added offices in Lexington, London and Louisville.
Saint Joseph Health Partners also provides care management services to the RETAIN Grant sponsored by the U.S. Department of Labor, supporting patients who sustain injuries or illness off the job and assisting them in getting back to work, back to work with accommodations, or training for new job skills.
Quality Highlight
With a focus on preventive screenings, cardiovascular diseases and diabetes, as well as patient adherence to the care plan and physician compliance with best practices, Saint Joseph Health Partners was recognized by America’s Physician Groups as a 5-Star “Exemplary” ACO for two consecutive years. SJHP has achieved $33 million in savings over five years in Medicare programs and $7 million in savings managing the commercial employee health plan. Participating in a three-year Medicare BPCI program, SJHP reduced readmission rates by 50%, achieving $4 million in savings with increased patient satisfaction and quality scores.
By working with more than 100 provider groups and nearly 2,000 providers in Kentucky, Ohio and Indiana, the network has collectively raised the quality scores for all of its provider practices. In 2017, the network earned an MSSP CAHPS/Patient Experience Quality Score of 73% and an MSSP GPRO Score of 70%. In 2019, Health Partners worked with six area hospitals to provide transition-of-care services for patients who are at high risk for readmissions to ensure continuity of care and reduction of readmissions.
